United Way Montcalm-Ionia Counties
Community Impact Grant Reports
United Way Montcalm-Ionia Counties recognizes the importance of collecting and utilizing data to help in storytelling efforts and in articulating the work that agencies do. This data translates into a valuable return on investment and verall impact on the local community. All grant recipients are required to report on three areas of data - Mobilizing Resources, Building Stronger Communites, and Changing Lives. In addition, grantees will be required to answer seven (7) Reflections Questions. Grantees have also been provided an optional Logic Model that is not required, but it is strongly recommended that it be used.
Progress Report Schedule


Grantees are required to submit four (4) Community Impact Reports on program outcomes. Please submit reports to Grants@liveunitedm-i.org

Instructions
1. This report is designed to be filled out and then added to for each subsequent reporting period.


2. Complete the Outcome Matrix.

a. Click on Outcome Matrix Tab

b. All grantees must report on all five (5) Mobilizing Resources metrics listed.

c. Under Building Stronger Communities and Changing Live, all grantees must select a minimum of five (5) measures to report on. A complete list can be found on the Program Outcomes Tab.

d. Under Additional Reporting Measures, grantees may report on as many additional measures listed on the Program Outcomes Tab as is reasonable for them to collect data.

e. UWMI acknowledges that grantees may already collect data and report on other outcome measures not included on the Program Outcomes Tab. Grantees may report on up to two (2) of these under Other Program Outcomes.


3. Answer the seven (7) questions listed on the Reflection Questions Tab as completely as possible.


4. Submit completed report by due date to Grants@liveunitedm-i.org. For subsequent reporting periods, please add to current reports, save, and email to Grants@liveunitedm-i.org.


5. A Logic Model Tab is provided. This is not a requirement for this grant cycle, but it is recommended that grantees attempt to use this model for planning purposes. It will be a requirement for future funding cycles.
